Job Title: Physical Therapist Profession: Physical Therapist Specialty: Contract Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:30 AM - 4:30 PM, occasional weekends if available Experience: 0 - 1 year of experience License: Current State Physical Therapy license Certifications: Member of the APTA Must-Have: Inpatient rehab experience Description: Performs initial examinations and provides physical therapy intervention to a diverse patient population. Works under the supervision of the Therapy Manager or Professional Practice Manager. This position is entry-level and part of a multidisciplinary team. Has the opportunity to rotate across inpatient, outpatient, and acute services. Performs initial examinations, establishes appropriate goals, plans and implements treatment interventions. Conducts timely re-examinations and modifies goals and treatment interventions accordingly. Coordinates with other disciplines when indicated. Assumes responsibility for all documentation requirements in a timely manner, including initial examinations, progress notes, monthly summaries, and discharge summaries. Provides training to families or care partners and medical personnel as indicated. Recommends appropriate durable medical equipment in coordination with team members. Writes letters of medical necessity when indicated. Participates in discharge planning and makes recommendations for continued physical therapy. Provides home exercise programs when appropriate. Attends all appropriate conferences, rounds, in-services, and meetings. May be assigned additional patients as dictated by staffing needs. Maintains therapy attendance records, patient charges, and statistics. Participates in the supervision of patient care associates, volunteers, and students. Must follow all policies and procedures.
