In this role, you’ll rotate across centers to welcome patients, coordinate check-in, manage scheduling, and assist with care coordination and outreach. You’ll help maintain accurate records, support referrals and transportation needs, and reinforce care plans and education. Working closely with clinicians, you ensure a warm, efficient, and respectful experience that reduces barriers to care and improves outcomes for seniors with complex chronic conditions.
