In this role, you’ll welcome patients, complete check-in, gather histories, and support clinicians with scheduling, follow-up, and care coordination. You’ll update EHRs, track referrals, and help patients navigate services with compassion and respect. Working closely with a multidisciplinary team, you ensure high-touch, timely, and accurate support that improves health outcomes, reduces avoidable hospitalizations, and enhances the overall patient experience in an underserved senior community.
